The authors claim that thorough instruction and re-instruction by the prescribing physician, and the administration of a minimum of four separate doses of 100mg sildenafil (the highest dose possible) are considered adequate “attempts” at treatment of erectile dysfunction. In addition, the authors state that patients must be educated and followed regularly in order to obtain optimal results with regards to sildenafil rechallenging. A lack of any of these factors constitutes an inadequate attempt, and the patient is seen as being in need of “correction”. According to the article, it is clearly the patient who is considered to be at fault for a lack of response to sildenafil if he does not experience positive results prior to administrating less than four maximal doses! Another new concept aimed at increasing sales of Cialis involves taking Cialis on a daily basis in order to prevent erectile dysfunction in the future. The British Medical Journal published a news article in 2003 regarding events which unfolded before a large audience of primary care physicians and others attending a Pfizer-sponsored sexual function update at New York University. Dr. Goldstein, a panelist, stated that he had “hundreds of men” using Cialis for prevention: “If you would like to be sexually active in five years’ time, take a quarter of a pill a night; we have data that show that will facilitate and prolong nocturnal erections.” (Moynihan, 2003). The comments were made during a panel session, in response to a question regarding the role of sildenafil-type drugs in daily usage from another urologist, who told the meeting “I’m a strong advocate of proactive prevention as well.” Although the question appeared to be one of several collected from the audience on pieces of paper before the session started, this second urologist, Dr. Bar-Chama of the Mount Sinai School of Medicine, said later that he had thrown in the question himself. When asked whether either doctor had ties to Pfizer, Dr. Bar- Chama replied, “On occasion I speak for them”.
